Using the wisdom and experience of primary and secondary practitioners in a Case Study methodology, this forum explored the key design elements, strategies, and approaches of an inclusive approach to transition. As the state prepares for the double cohort primary-secondary transition, how are we as primary and secondary school leaders working to maximise the engagement, inclusion, and well-being of all students, particularly, those identified as being at risk?
